OSA Welfare Fund Health Benefit Award
The Organization of Staff Analysts' Welfare Fund's Trustees have voted to send a Health Benefit Award to each member who has received the vaccination by November 30, 2021. The Award will be accompanied by a $500 check to each active or retired member of the Welfare Fund.
How to file a claim
- Complete the OSA Health Benefit Award Claim Form
- Attach copy of Covid-19 Vaccination Record Card and/or Excelsior Pass print out with details
- Sign and submit it to: Administrative Services Only, Inc on or before November 30, 2021
